Any patients requiring information about testing please go to the government website: https://www.gov.uk/apply-coronavirus-test. Patients who do not have access to or cannot use the internet can call a dedicated phone line, which can be used to order a test if no other options are available. For COVID-19 inquiries please use NHS 111 on line or call NHS 111.
If you require a sick note for self-isolation go to NHS 111 online to obtain isolation note. The practice can not issue sick notes for any COVID-19 related sick leave

Disabled Access
There are two reserved disabled car parking spaces near the front entrance. Wheelchair access to the building is via a ramp near the front entrance.
A disabled patients' WC is also available. If access proves difficult to any of our disabled patients please inform our staff.

Please turn off your mobile phone before entering the medical centre, as this disturbs staff and patients and may also interfere with medical equipment. Please let us know if you change your mobile number.
